The short answer to your title is that Donald Trump has many negatives ahead of the age issue.
However, Biden's age is an issue because it's a tangible and irrefutable fact and can be used in place of more subjective criticism.
His eighty-one years are not the real concern; voters don't particularly like Biden. Two-thirds of Democrats didn't want Biden as their nominee. Iowa and New Hampshire, in the 2020 primary, dismissed Biden as old and lackluster -- "Dead man walking" was CNN Van Jones's phrase.
That said, age affects ability. Biden communicates poorly in interviews. Just recently, he had jumbled replies to MSNBC Jonathon Capehart. With Fareed Zakaria (a few weeks ago), he fared poorly, and the press conference in Hanoi ended prematurely because of his performance.
