The Bolshevik takeover of the Royal palace in St Petersburg had problems.

Several military units were needed because the previous ones had located the wine cellar where a fire had broken out. Dousing the flames took several teams of firefighters because they, too, got drunk. The flow of wine was so great onlookers were collecting it from street gutters.

I suggest you read "Blundering on the Brink" in Foreign Affairs—decision-making by too few without proper information and criticism.
